Judging by the realities on ground, Health insurance has come to stay. Either through the NHIS or the plans
promoted by different HMO's, Nigerians are getting used to being covered by one plan or the other. Different
State Governments are also getting set to introduce their own health cover for their indigenes and other people
living within their domains

However, experiences have been varied and though many have commended the service quality of most of the HMO's,
some have had bad experiences they really should not have had.


Lets get the ball rolling by reviewing the nature of coverage and quality of service rendered by participating hospitals
on behalf of HMO's. If you have had good experiences, please share and if they have been less than pleasant, also
share. Also do we indeed need Health insurance? I believe we do and everyone should become part of the drive to
secure the Health of all citizens in the country and reduce mortality figures.

Hi, had internet downtime yesterday, so here comes the follow up to earlier discussion.
I intend to make a case for Health Insurance by pointing out its benefits, disadvantages and
solutions for the majority of the population of Nigeria.

Health Insurance (Managed Care) is the provision by a Health Maintenance Organisation of a defined
set of healthcare services to an enrolled population at an affordable cost by a network of selected
healthcare providers. As such a person enrolled can have immediate access to healthcare services even
without any extra expense of cash at designated centres.

This is critical, especially in emergencies, when one is cash strapped. There have been several cases
of people involved in accidents dying for want of adequate care at Hospitals visited simply because
they lacked the ability to pay for prompt care. One would not want an unprepared for expense as
low as N2,000 required as registration and consultation fee at tier one Hospitals to lead to a fatality.

It is in such a case that health insurance becomes critical, as it provides a hedge against such an occurrence.
Like all insurance products, the beauty of the program is the pooling of resources in order to share the risks
faced by many.

Question 1

What if despite owning a health insurance plan, one is still denied care at health facility due to bad word of mouth
or previous inability to pay incurred liabilities by HMO?

Apply due diligence in inquiring about the financial fitness and integrity of the HMO you would want to purchase a
health plan from. this is why previous testimonies or tales of disappointment would be useful. So if you have pleasant

The second of twelve features / benefits of having an HMO Plan is the modulation of associated costs.

Individual visits to a health facility attract fees dependent on the the perception of your net worth by Medical
personnel. The same ailment treated at two different but similar health facilities will result in two different
bills being issued.

With an HMO plan, similar tier Hospitals charges are subject to charge at similar rates. As such once you
keep attending the same health facility, you can expect bills that can be taken care of by your plan with your
HMO.

Three years ago (2015) before i took a plan, my two year old son had pneumonia that resulted in admission in a
popular Surulere Hospital. He was there for five days. My bill for his treatment was N54,000. At the time
of the incident, I was out of cash and was only able to handle the payment through installments over three
months. Another child admitted in the same hospital was billed N200,000. I guess my bill was lower because
the Medical Director was a member of my church.

A basic health plan of N25,000 would have covered me in that case even if the bill had been N100,000.
It was later that I got to know that even the N25,000 could be paid in instalments of N10,000, N7,000 and
N8,000 over a space of three months.

Question

What if I pay N25,000 and do not fall ill at all during the 1 year plan period?

Answer
What if you do fall ill and are cash strapped like i was and do not have the luxury of knowing any Medical
Director? The opportunity cost is the lost man hours and inadequate care if all you can do is attend a
lower tier Hospital with lower expertise available. Also you do not have to wait till you have a major illness
like pneumonia before you buy a plan


It is better to accept the responsibility of protecting your loved ones by hedging against emergencies.

Different Benefit Plans

Continuing from Yesterdays discussion, another feature or benefit of having a Health Plan
is the ability to choose level coverage or consequent exposure. Since net worth of individuals
varies, several plan levels are catered for.

A Basic plan covers general consultations and prescriptions, minor surgeries, dental, optical and
Gynaecological care, tests and any physiotherapy required. Emergency evacuation from accident
scene is also catered for. Due to these provisions, a maximum limit of N400,000 can be incurred
for a plan costing N25,000.

Individuals of higher net worth will enjoy greater benefits for higher plans. To enjoy annual
medical screening from top notch facilities equivalent to those provided for overseas visits
and also cancer and dialysis treatment for the first three sessions, a yearly sum of N175,000
is needed. For such a sum, maximum benefits is N2,000,000.

These are worst case scenarios which no one wishes for but which some will definitely face.
A responsible family man or woman would normally situate themselves somewhere in between
the two plan levels.

For whichever plan however that one undertakes, a higher level of care than otherwise available
would be at ones disposal with opportunity for choice between two or more facilities.

24 Hours Nationwide Access

Just a word. You are alive for 24 hours a day for as long as you live. You therefore need to be protected
from ill health 24 Hours a day any where you are. As such you will need to be protected
24 Hours of the day Nationwide.

A health plan by a National HMO will ease any worries in that direction. Most HMO’s although
headquartered in a city will have branches and agencies all through the length and breadth
of the country.

This gives you the ability to make use of Hospitals in case of emergencies in any major town
or city Nationwide, if you are someone given to undertaking journeys or are holidaying in another town.

Since your plan determines breadth of treatment you are entitled to, your choice of expertise
required is wider and can afford you taking treatment in a top notch Hospital where you are visiting.

Overcoming hindrances to consultation.

Since you now have a plan, the usual hesitance to visit the doctor for complaints would be
removed and you will feel freer to seek Medical Intervention when signs of illnesses appear.

Fears of high Doctors bills would be removed and this would lead to early detection and proper
treatment of ailments. You can also seek permission of your HMO to go for a second opinion
from a Specialist.

In all, a greater breadth of control is now available to you. All these for no more than N2,000 per month
paid semi-annually.

It is all about facing risks with a positive attitude and a good plan. Hiding away from a risk factor does not
remove the risk, rather it leaves one with a greater exposure.

Would have made all the difference if the man had an ID card from an HMO. A call would
have listed hospitals around that could be visited within the hour.

Even though the money is an ever present issue, it should not have caused the mans death!!

Lets take the advice and get secured, hedge against unnecessary risks by taking on a plan.

My thots,

A Medical Professionals career starts with being idealistic and as desired patriotic to the Hippo Oath.
What changes however is the environment in which Medicine is being practiced in Nigeria. As stated earier
Government's concerns are too numerous to commit required resources to needy sector. The treatment
meted out to Health is the same meted out to Education.

Which is where the private sector comes in. However the main concern of the Nigerian entrepreneur is a fast
break even and profit potential is uppermost, which is why many businesses die within two years of inception.

The doctors originally cited for impatience may have acted out of two possible motives - to attend to a long queue
of patients within time allotted without exhausting him(her) self, or because he had quantified what his time was
worth and didn't think any patient deserved more than that.

Whichever motive is paramount, it still remains a fact that money plays a great role and Governments can not be
left with the responsibility to ensure a system built on Quality control and assurance.

More money is needed for the system and i believe that the role of Health Insurance (Managed Care) is essential
in creating a pool of funds that can drive the system. cool

I am a Nigerian doctor, and I work in Nigeria.

I have been on call, 24/7 for the last 365 days, no weekends no leave. Why? Because the community hospital where I work in cannot afford to pay many doctors at the same time. Why? Because the patients cannot afford to pay for basic healthcare services. Why? Because the economy is bad. And if they can't pay, how can you give them QUALITY healthcare?

I handle emergencies with rechargeable lanterns.

I site intravenous lines with my phone torchlight.

I consult in a room with no fan, let alone an A.C, and I am expected to rack my brains and make diagnoses, even though I see a large number of patients within a short period of time.

I have had to operate on patients with phone torchlights.

I have had to improvise an incubator with a CARTON for a premature baby.

I have had to use a suction bulb, instead of an AMBU bag or an oxygen delivery apparatus, to resuscitate a baby.

I have had to use 5 ml syringe to resuscitate a patient in shock because there was no infusion set [compare this to using a spoon to fill a bucket with water]

I have practiced for 3 years but my bank account doesn't reflect it. Why? Most of my patients are poor and I am forced to pay for their treatment. Fact!

I have managed diabetic crises without hourly RBS, let alone tests like SEUCr [compare this to sailing without a compass, or flying without a radar]

In the western world, CT scans, MRI, ECGs are obtainable in the emergency department. Largely because there's an effective health insurance scheme. But here, my patients can't afford 3 square meals, they cant even do low cost tests like FBC, MP and PCV [each of which costs approximately #500], so how can they afford CT scans and MRIs, most of which costs as much as 50k to 80k for a single test.

How do you want me to diagnose them right when I cannot do these tests to confirm my diagnosis? By magic or clairvoyance?

I am faced with these daunting challenges. And I am faced with two choices: to watch them die, or to use my brain, my raw clinical acumen, and improvise to make ends meet. Most times I get it right. But sometimes, who knows, I may not. Why? Because I am no bleeping "mutant X". I got no x-ray vision! Neither do I have a magic wand.

So next time before anyone compares me, or any other Nigerian doctor, with the average doctor in the western world who has all kinds of " medical toys" at his beck and call, the person should do well to bring such a person and place him on a 24/7, 365 days call in this shitty hell hole and compare his mortality rate with mine. Only then can we have this talk.

Yes, there are incompetent Nigerian doctors [ I am working on an article on that ], but so there is in any other profession, both in Nigeria and in the west. Yes, even in the Western world! John Hopkins recently published a study which revealed that the third commonest cause of death in the U.S of A, 3rd only to Cancer and Heart disease, is doctors' error! That is in the United States.

So, contrary to what we may think, "misdiagnosis", "negligence", "incompetence" is not peculiar to "Nigerian doctors".

We should stop blaming "Nigerian health care workers" and fix our system. The government has a lot of work to do. And until we do that, it would be unfair, perhaps inhuman, to compare our health indices with that of the western world, and to blame our healthcare workers for supposed "incompetence", when it falls short of what is obtainable in the western world.

I want a situation where there's an effective health insurance scheme in place, where the average man in the street can afford basic healthcare delivery, where I can run as much lab investigations as I want without being afraid that the patient may not be able to afford it, where every patient has an ECG monitor by his bed side, where I can do CTs and MRIs even before taking full history, where I wouldn't have to improvise incubators with cartons, where I wouldn't have to operate with phone torchlights, where ......

This is what I want. But I don't have that luxury. So I try my best to make ends meet in order to save lives. You should really take a walk in my shoes.

I am a Nigerian doctor, I work under impossible conditions and still generate positive results. I walk with my head held high.

Children in focus

It is sure that your children are highly cherished by you. They deserve prime position in the
consideration of a Family man or woman. As children they are exposed to risks of injuries,
due to their playful nature as well as childhood illnesses that are common at childhood.

Although their schools generally have contacts with or relationship with nearby Health facilities,
they may due to cost choose lower tier hospitals or those without Pediatricians. In some cases
this is critical, as there are some ailments only the trained eyes of an experienced pediatrician
can detect.

A case in point is that of a 7 years old girl who kept complaining persistently of stomach aches.
She was taken to a hospital, where drugs were prescribed, but when the pains subsided and
reappeared, medical advice was sought from a specialist at a Government secondary health facility.
The Doctor said he diagnosed a hole in the navel and that a surgery would be required within a month.

Just as a check before surgery, the girl was taken to LUTH, where after examination and some tests,
the Pediatrician stated that there was nothing wrong with her. Believing the Second Doctor would
have meant an unnecessary surgery and exposure to its unwanted dangers. Obviously, sometimes
you get what you pay for.

For simple illnesses, routine consultation and any minor surgeries, most HMO’s have basic plans that
cover kids at a lower rate. This rate is also available for schools insuring kids and including the cost in
school fees. It is actually beneficial for schools to make a provision for health insurance to the tune of
between a tenth and twentieth of school fees, thereby affording kids Managed care even when they are
not covered by a family health plan.

The difference between Life Assurance, General Insurance and Managed Care (Health Insurance)

Life assurance plans generally provide for benefits to dependants in case of death of person insured,
while General Insurance caters for unforeseen risks in the course of the daily experiences of those
insured. In most cases, there is no payout except in the rare case that the risk plays out as a danger
or fatality or loss to the person insured.

There is a little difference in the case of Managed Care (Health Insurance) which could be considered
as a savings for an eventual rainy day. Even where there is no heavy downpour, a slight drizzle is
catered for immediately. As such even without the clear signs of dark clouds, meaning that rainfall
cannot be predicted, minor occurrences are catered for.

While under coverage, any signs and symptoms whatsoever can be properly diagnosed and treated
before getting out of hand. So from day one you will be entitled to treatment if covered under Managed
Care. As such there is a contingent liability on the Health Maintenance Organisation to make provisions
for your care at facilities designated for your care.

In order to predispose health facilities to immediately attend to HMO clients, a certain percentage of
annual plan fee charged clients by HMO is prorated and paid monthly to clients designated health facility.
This payment is called capitation.

As such, whether the client uses the health facility or not, the Healthcare providers are paid in advance
of any visit. If the client actually then uses the services of Healthcare providers, any charges incurred
are then settled by the HMO.

There is a minimum level of benefit enjoyed by HMO Clients that is not enjoyed in other general cases
of Insurance. The exception perhaps might be savings inclined Insurance plans like those that provide
for Education through payment of premiums.

Thus a client undertakes to forego amounts paid in excess of capitation in exchange for the right to
enjoy higher amounts than premiums paid in the event of the incidence of ill Health requiring such bills.

The Health Insurance system provides for a base income that assures Hospitals of continued patronage
and moderates their need to overcharge occasional patients to cover for expenses. This stability enables
greater effectiveness and efficiency in relationships between clients, HMO’s and Healthcare providers.

Q Are you sure I can get all the benefits listed under my Health Plan by paying just a token
as a premium for a whole year?

A What is paid as premium is usually sufficient to cater for all reported bills incurred by enrollees
who report sicknesses. The applicable rates for HMO plans are not decided on by fiat, but are a result of
medical data, applicable fees charged by Hospitals and health facilities, health index of the population segmented
by age, cost of operations etc as determined by Ministry of health, Economists, Hospitals, actuaries, statisticians and case

When the dynamics of the HMO market have been fully understood and allowed to determine rates, it will be difficult
to run foul of the game. HMO's fail due to several reasons chief of which are lack of technical sophistication or
maladministration.

You can take the premium as an average amount charged for services provided under each plan. If arrived at meticulously, and if the HMO is well administered, it is difficult, even highly impossible to go wrong.

The Human Resources Manager in a corporate organisation prior to the promulgation of the NHIS Act would
do well not to act in the dark but was limited in his evaluation of case histories of employees reported to him.

Q . Are my pre-existing conditions immediately taken care of by my plan?

A. It is important that a note of caution be made about pre-existing conditions i.e medical
conditions a client may be suffering prior to taking a plan. At the point of enrolling for the plan,
several questions are asked as to the state of health of enrolees to ascertain if they are subject to
some common ailments requiring continuous medical attention that would imply continuous medical
bills. In a case, where there are such conditions, the plan under which insurance will be provided is
varied by the need for underwriting the other than usual expenses that will be incurred.

The essence of an insurance policy is the cover to a certain sum for minor and major ailments upon
the payment of a sum called the premium which though compared to the cover is small but when collected
from several clients takes care of occurrences of both minor and major incidents while leaving the insurer

For the Company and its staff

1. Reducing absenteeism. No employee can claim he was unable to visit a retained Hospital due to distance
and therefore had to stay at home to receive treatment.
2. Fraudulent bills. The era of such bills due to collusion with Hospital staff will be put to an end.

There are many instances where staffs of Companies are absent from work for short or extended periods and
they claim that absence is due to their ill health and inability to visit retained hospital due to far distance from
their homes. This will be prevented as there will always be an HMO designated Hospital within reasonable distance
from most locations.

The relationship and bond between HMO’s and designated is a strong one and assures that cases of fraudulent
bills due to collusion with Hospital staff are brought to barest minimum, ensuring that corporate organisations
only pay for services they enjoy.

Medical facilities define their target clients and bill for services based on perceived net worth of clients and ability
to pay. Excessive billing for regular services are bound to be queried by Medical staff of HMO who are there to
ensure efficiency of the process and that there are no instances of excessive bills.

Any staff involved in accidents requiring minor surgery are immediately attended to by Hospitals without recourse to
billing process and billing is sent to clients as an after the fact activity.

To illustrate, staff of my HMO were on a Marketing trip, when after stopping along the way and opening the door of
the car, an okada rider swerved to avoid a pothole, and almost severed one of his toes on hitting the door. The toe
was dangling and the pain was much. My colleagues called at a designated hospital where the doctor on duty refused
the case referring them to Igbobi Orthopaedic Hospital. It took them calling the Help Centre, to contact the Medical
Director who after correcting the Doctor on duty proceeded to stitch the toe back in place since the bones were not fractured.

The resultant effect of corporate enrollment is for the company to have a retainership agreement with many Hospitals at
a low cost.

Hello,
I was just scrolling through some HMOs websites and decided to check nairaland to see if our people have had experience with the health insurance. it is a good one here.
i work in a company where i am open to a free medical care only at the companys clinic(though when reffered for tests, the bill is covered). i plan to have a health insurance for myself so i can have access to a wide range of hospitals outside.
my questions are: will a card or some means of identification be given after subscribing for the plan?
2. will it cover major treatments incl. admission and drugs without any other payment?
Been checking mansard and hygeia....and honestly they have good plans. which is better?

To attend to @mrmichael1's inquiries on Health Insurance,

- a Health Insurance plan through the HMO I recommend will avail you with an Identity card that allows
you to receive accelerated access to medicare at your designated provider and in case of emergencies at
nearest hospital on our network. All you have to do in case of an emergency is to call the 24/7 call centre
number and you will be directed to the nearest network provider.

- If you are interested i can send a Table of Health Insurance benefits listing all the care provided, but suffice it to say that
with as little as N25,000, you can be protected with treatment against any occurrence of some debilitating illnesses
as appendicitis and hernia and other minor procedures. At some centres, appendectomy could cost as much as
N80,000. Please send your email.

- Both Axa-Mansard and Hygeia are major HMO's with good track records but are biased towards different
segments of the market. Hygeia caters for the major cities with preference for the high end of the market. Non Price
sensitive and High net Worth clients are preferred. Their plans are rather pricey. Hygeia also covers some aspects of
Travel Insurance by offering Global coverage while Mansard covers clients in Nigeria and India.

- At a later time I will dwell on a comparison of HMO Plans by 5 of the foremost HMO's looking at their slants towards
different segments of the market to enable readers reach an informed decision on the HMO to favour for each Plan
configuration.

Like you said, Health Insurance is a good possession. Again like you said it is good if you can afford it.

HMO's have over the years, with increasing acceptance of their products, tried to make its possession
cheaper to afford. HMO 004, that I represent now allows for payment in instalments. A plan costing N25,000
can be started with a payment of N8,000 and the balance paid in four to six months. Look at that figure in
comparison to the benefits which include exhausting the premium and incuring bills to a maximum tune of
N430,000. NO Insurer looks forward to all insured exhausting their claims, but that is a technical possibility.

The National Health Insurance Scheme beneficiaries pay sums in that range and have to make do with expertise
found in Government Hospitals and primary health care centres. HMO's mostly use Private High Quality Hospitals
and Consultants not working under stress of a high patient turnout.

It should be noted that without the assistance of HMO's moderating bills drawn up by Medicare providers, it would be
difficult if not impossible for a middle class income earner to afford medicare from hospitals the quality of Eko Hospital,
St. Nicholas and Lagoon Hospitals. Though you have to pay more premium to afford that kind of care, it has been
brought within reach.

It is good that you have found a way to deal with anxiety using CBD oils, but one problem with traditional or alternative
medicare is the issue of dosage and side effects. Some others have found solace in meditation while others have found
smoking or drinking helpful. I depend on God's grace and medical assistance.

It is the noticeable gap in communicating with and alleviating problems of potential and actual enrollees that gave rise to
this topic and i would like to use this medium to correct some of those ills. So if you need info or have had a good or bad
experience with any HMO including HMO 004, please feel free to use this forum to vent it.

Hi guys. Health Insurance is now a buzz word in the Centre of excellence.

Was at the launch today of the Lagos State Health Scheme (LSHS) at the Civic Centre, V/I, Lagos
At the Launch were the Lagos State Governor, APC Candidate, The Oba of Lagos, The Ayangburen of IKorodu,
the Oloja of Epe, several White cap Chiefs, The Ambassador of Netherlands, Representatives of the World
Bank, USAID and the Host, the Commiksioner for Health of Lagos State.

Details of the Health Insurance Scheme were Shared and the State Governor was the first to be enrolled.

With the launch, it is now mandatory in Lagos to have one form of Health Insurance or the other i.e. you must
be enrolled with NHIS, LSHS or HMO Registered.

HCI, Healthcare International (HMO 004) is in charge of registration of LSHS enrollees in Eti-Osa, Ibeju Lekki, Ikorodu and Epe.
Other nine HMO's have similarly being given locations to cover. This shows the level of confidence, Lagos State has in the
services of these Ten HMO's.

As a reminder, HMO's provide enhanced cover through top notch Medical Facilities. I would encourage you to pick up your
