Good morning, I humbly seek the input of professionals i.e Doctors and Nurses in the house.

I am in my early 30s, married and I have 3 kids. For some months now, I noticed I have been having abdominal pain, left side to be precise. There was a time I went to a Catholic hospital to complain about the tiny pinching pain that goes and comes back.

I was given some antibiotics, the pain left but still came back. I changed my hospital to another one, so on Friday I decided to complain to another doc at my new hospital and he pressed my stomach, I didn't feel any pain but when he hit my lower back gently with a fist, I had little pain. I was asked to do a urine test, and the doctor examined the result and said it was fine and okay. He said he will have to treat me for bile load nephritis or something. I asked if it's something serious or I should be worried, he said no.

He then requested I do a total abdominal scan to do a thorough examination, which will be done soon. Then placed me on flaggyl, gentamicin injection 3 per day for 3 days,vitc 1000g, doxycycline tablet. I will finish the injection today. What really scared me was that he asked, are you married, I said yes. Then he said your wife will have to be treated too.

I want to tell you that the pain has reduced even before finishing the dugs and injections, but I am really worried, WHAT I'M I BEEN TREATED FOR?

You have the right to ask the doctor that and be answered. The answers you will get here cannot be compared with those who have access to your test results. Go back & ask the doctor to explain to you in layman terms.

For him to give you, doxycycline, gentamicin and flagyl which are heavy antibiotics against infection, He supects an infection, and u have contracted your wife as well.

Some patients do not bother to ask. Some do not even bother to ask the about the drugs prescribed for them by the Doctor.

@op, we cannot guess what is wrong with you. You've been to the hospital, ask your Doctor what your health challenge is. It is your right to be told what was found from the Lab tests carried.

The doctor who is presently seeing you isn't doing bad. His differentials are apt and he needed the urine test to nail 1 exactly but it wasnt definitive, still he advanced.
His goof here was not briefing you on your state of health and give you sufficient info. Meanwhile, it is your right to ask.
He most likely mentioned pyelonephritis (not bile load nephritis). So, next time you go there for injection, seek his audience and confirm this. Also, ask him whatever might be bothering you.

After every consultation, you are entitled to a briefing on a doctor's assessments and line of care, then a room for you to ask questions. This may not be possible in most govt hospitals in Nigeria because a doctor is seeing patients that 10-20 doctors should be seeing. There are more than enough patients waiting outside to see him and such time for talks would be enough to reduce the queue. This is sad really but that's the sorry state of our system.

On his question about your marital status, your sexual partner(s) can contract the stuff from you and he is trying to prevent re-infection. A case whereby ur wife might have been a host to it already. If he treats you alone, you will go back home to carry it again. Afterall, no one asks a married man to give up his marital "snacks".

However, I will add that his target can be achieved if and only if you do not have women you're "seeing" apart from your wife, because if you do, you're at constant risk of contracting it again and again.

The doctor actually informed the Op about his condition but apparently there was no communication.

The doctor told the Op he was treating him for pyelonephritis but the Op interpreted it as bile load nephritis and he (the Op) did not border to ask what the hell that meant and the doctor also forgot he was taking to a layman and not a colleague of his.

Communication is the key, its importance cannot be over emphasized.

To the Op also, there is no infection you contracted from your wife neither will you give it to her, no.

Pyelonephritis is a type of bacterial infection that affects where urine is formed and the passage way of urine to the bladder. That is why the doctor asked you to do urine test (which I presume is urine culture).

So with antibiotics you will get better and your wife does not necessarily need to be treated except if there is any other reason we don't know yet.

What I do not understand however, is the rationale for including gentamycin in your treatment.

You guys as patients/citizens should also examine your roles in that "failed health care system", as you're all contributors to it.
What other diagnosis/ disease entity does an average Nigerian know of, apart from malaria and typhoid?
What other disease entities does an average Nigerian understand fully well, and can discuss meaningfully about?
How many care to ask what they're being treated for, or even listen when it's been explained?
The average Nigerian is completely backward and ignorant when it comes to health issues, compared to the Whites and Asians I've met.
See how the OP was told "Pyelonephritis", but he couldn't even comprehend or pronounce it well because it didn't sound like malaria or typhoid; instead he wrongly went for "Bile Load Nephritis", and didn't bother to seek for clarification from his doctor or hit Google for the right spelling!
Asians will immediately Google it in your presence, after asking you to spell it.
I recently met an Indian patient in the UAE who came for reconstruction of his deformed fingers, and he completely schooled me on his medical problem: Rheumatoid Arthritis.
Immediately he sat down, he boldly told me he's a Rheumatoid arthritis patient, and went ahead to detail me on every thing about his condition, including the names of the various medications(Prednisone, Methotrexate, Hydroxychloroquine, Sulfasalazine) he has been taken, their dosages, side effects, and how often he does FBC, LFT, and electrolytes. And I was highly impressed.
A typical Nigerian doesn't even know how Rheumatoid arthritis is different from Osteoarthritis. Them and their Herbalists term both as "Rheumatism".

Nigeria's problems are so many, and are contributed by all of us.

With Gentamicin, I guess you are being treated for infection and it is always good when both of you are treated at the same time. That gentamicin is a strong antibiotic. It can ease one week infection pain in less than an hour of taken it.

First, sorry for keeping away from the thread for so long. The drugs has really weakened me and I just too lazy to type. Last night, I had serious headache as if my head will fall off.

Let me try to respond.

Thanks for this, I have read the info about the drugs prescribed. And the infections they treat are majorly sexually transmitted infections like staphylococcus, even gonorrhea. I also read that staph is all around us and if our personal hygiene isn't top notch, we might easily get infected without knowing and if left untreated could be dangerous. Thanks

I understand it must be an infection. I am just baffled, what infection exactly? So that I will then ask myself, how did I contracted such? For your last paragraph, I explain what my complaints was to the Dr and he prescribed drugs. I have to trust my doc and he is the professional. But he is the only Dr. on duty in a big hospital. Corona virus social distancing and covering of nose as well as the fact that other patients are waiting at the door makes it difficult to be really interpersonal, more so, he sat at about 3 to 4 feet away from me. And had hesitations leaving his seat to tap my back. He eventually did cos it was necessary and he was quite fast about it.

Those drugs are very strong, kai! I could only manage to go to work yesterday. I have taken the week off to allow the drug work perfectly. The pain has reduced, I suspect the infection had been with me for a while and the Dr saw the need to bombard my system with the antibiotics to cure it wherever it has gotten to. My wife will get treatment soon as well. I have told her everything, she was aware of my abdominal pain and I told her I will be going to the hospital when I did. Thanks

Everything you said is correct, as if you were there with me at the clinic. So, just let me tell you about the effect of the drug and the scan.

The drugs are very strong drugs, this my first time seeing those drugs, not to say using them.

I wonder till now, what particular infection I got that warranted those drugs, and how exactly did I get it Or where? But right now, which the comments I have read, it does not matter, I just want to be fine and my wife fine as well.

About the scan, I went to where I was referred to yesterday for the total abdominal scan, but they could only register my letter and asked me to come at a later date. I will also give a feedback when its done.

There is a big work load for the Dr. But its important to inspite of the load to inform patients about what is making them sick, so that they can take precaution and take more responsibilities. Some of us who are not so informed about the field of medicine may be at a disadvantage in this. Thanks alot

I totally agree with you, cos till now, its still a mystery. Before the antibiotics were prescribed, I was asking myself is it ulcer? How can a glutton like me who don't joke with food have ulcer?
