If you are applying for a new passport for the first time, there is no need to get a guarantors form or even physical copy of passport photograph. Just carry your application form and the form created for your interview and head to the Immigration office on the day you scheduled for yourself and the venue that you selected. In my case, it was HQ abuja and the timing was irrelevant, as I came by 10 instead 12 as my schedule said and i was still allowed inside. I left by 1.20... Line was much there.

Please Guys, I had a little network challenge while applying for my passport and on submission I saw my LGA was different and my state was made to be Abuja. I have called all their CC lines and no one picked.
Who has had similar issues and how dis you resolve it please? My capture date is 14th February tho.

I have the same issue, am in lagos and my appointment is scheduled for 19th Feb. My state of origin was replaced with my current location seems the their site has some issues.

Went to the immigration office on my captur day and they corrected it by themselves for me.

BUT I HOPE THE GUARANTORS FORM WASN'T REQUESTED FOR

so we shouldnt bother with the guarantor form and proceed with documents uploaded and proceed for the appointment

You can staple the application slip to the receipt (Application slip on top). Also go through the list of documents on the appointment slip(The ones that apply to you). Ignore the Guarantor's form, it's not required.

Are you sure you can't use your NIN? Where did you get the information from?
