Some men can be very wicked or very strict and during their old age, everyone runs from them. I know one lawyer that hasn't spoken with a wife he has 5kids with for 10years . At least he must have had a staff,even if its one if the clinic was still running.
Even if it was no longer operational,did he not get calls to his phone,did the neighbors not notice lack of any activity and didn't perceive the stench.
